I have four children, and went through a great deal of difficulty through having them. I'm also on medication which would affect any baby I would carry. When I think of not having any more children, my heart breaks, but my husband has had a vasectomy because he thought I had already suffered enough. He is not Catholic and I am on only still on a journey to Catholicism. Would this situation pose any problems in the future - am I in a sinful position that might mean I couldn't take communion for example. Any wisdom appreciated.

My prayers for your situation. It is hard. You should make an appointment with your parish priest to discuss your specific situation. But the Church does not mandate that you have children for as long as you are fertile. For grave reasons, and difficulties with prior pregnancies and medication you are taking would very likely be considered grave reason, a couple can postpone (even permanently) future children, using licit means. Regarding your husband's vasectomy and the state of your own soul, I don't want to worry you in any way but am encouraging you to speak with your priest in private about the matter, where you can be reassured with certainty.

Thanks for your prayers. Now I just have to pluck up the courage to speak to a priest which is something my husband would be dead set against! Pray for me in that regard also.
